This article reports the development of a prototype peptide enzyme immunoassay for B. microti-specific antibodies and its preliminary performance characteristics in sera before and after infection in 59 individuals with known tick-borne infection. The reactivity showed a high correlation with B. microti immunofluorescent assay and immunoblot, suggesting that the peptide enzyme immunoassay may be a suitable candidate assay for development into an automated blood donor screening method.

This article assesses the seroprevalence of antibodies to these two tick-borne agents in blood donors in Connecticut and Wisconsin and demonstrates that a history of tick exposure does not predict higher seroprevalence for B. microti. It also demonstrates by donor survey that recent exposure to ticks is reported by up to 9% of blood donors in geographically representative blood centers.

This is a case report of apparent transfusion babesiosis after a heart transplant in New York City. One of 32 red blood cell donors had a positive antibody titer for B. microti, but was negative by PCR and hamster inoculation 5 months after their donation. Serum from the heart donor, a lifelong New York City resident, was unavailable for testing. The case illustrates some of the difficulties in establishing a transfusion cause for the infection.

This article reports the second reported case of transfusion-transmitted WA-1, occurring in California from a donor infected during a camping trip in Central Oregon. Transfusion babesiosis with B. microti in premature infants has been previously reported. The case report illustrates the use of hamster inoculation to isolate the responsible piroplasm, to exclude the mother as the source of the infection, and to identify an identical organism in the implicated donor.

Transfusion 2003, 43(suppl):13A. This abstract reports on Lookback on the recipients of recent blood donations from 54 blood donors found to be seropositive for B. microti. Twenty-six percent of tested recipients of red blood cells and 22% of all tested recipients of cellular products were found to be seropositive. Such identified recipients should be evaluated for parasitemia and for clinical symptoms of babesiosis and should be appropriately managed.

This abstract summarizes 18 reported cases of transfusion babesiosis during an 11-year period in Connecticut. The cases represent the transfusion of approximately 1.8 million red blood cells, resulting in a reported incidence rate of 1 clinically recognized case of babesiosis for every 100,000 red blood cell transfusions in this endemic area. The majority of the cases could be traced to a specific blood donor, although in three cases no donor was identified (and could represent tickborne babesiosis), and in two cases the investigation of all donors could not be completed.
