The impression that elderly patients have long and effective regional anesthetic blocks was tested by studying the duration of a mid-humeral brachial plexus block with 0.75% ropivacaine in elderly patients. Indeed, a significant positive correlation with increasing age was found. The reason may simply be a general deterioration in nerve function, involving increased sensitivity to local anesthetics in the elderly.

The so-called supraclavicular 'pumb-bob' technique for blocking the brachial plexus has by tradition been considered safe and to give good results. Both pleural and intravascular complications, however, have been reported. Therefore, the present authors performed an MRI study and showed that the needle should actually be directed 45° cephalad, instead of in the anteroposterior (perpendicular) direction, in order to avoid pleural and subclavian vessel puncture.

There are very few large-scale studies on complications of continuous regional block analgesia techniques. The present authors report an incidence of 1% of neurological deficits in the arms of patients who had continuous axillary brachial plexus block analgesia. Some of these complications were clearly not related to the plexus catheter. Due to the retrospective nature of the study, the causes of most of the neurological deficits remain obscure.
