J Vasc Surg 2002; 35:137-145. This is the first paper to demonstrate increased macrophage density in echolucent versus echo-rich carotid plaques. Increased macrophage density was also associated with increased plaque lipid content and elevated levels of plasma and LDL cholesterol. Finally, aspirin use was associated with reduced macrophage density.

The paper demonstrates in a post-hoc analysis, and, for the first time, that benefit from a multiple risk factor intervention programme is confined to those patients with echolucent carotid plaques. No benefit was observed in patients with echo-rich plaques or in patients without carotid plaques. These data should, however, be confirmed in a large-scale trial.

These observations were more pronounced in women than in men.
