Fifty-six confirmed PE patients were enrolled prospectively in this study to evaluate the prognostic value of troponin T levels. This was one of the first studies to show that increased troponin T levels were associated independently with mortality as well as prolonged hypotension, shock, and need for subsequent resuscitation. This study lends credence to the presumption that cardiac troponin levels may be used to stratify PE patients by risk.

Troponins were evaluated in 106 confirmed PE patients. Troponin level correlated with echocardiographic RVD, mortality, recurrent PE, and hospital course. This study adds strength to the argument that troponins can be used for risk stratification of APE patients.

Troponin levels were evaluated in 36 PE patients in this randomized, double-blind study. The authors found that 39% of these confirmed PE patients had an elevation in troponin I and that these patients were more likely to have more segmental defects on ventilation-perfusion lung scanning as well as right ventricular dilation by echocardiography.

Circulation 2000, 101:2817-2822. This study evaluated 209 patients with confirmed PE, finding that 31% had submassive PE. This subgroup had a high rate of subsequent shock (10%) and inhospital mortality (5%). Echocardiographic evidence of RVD in otherwise normotensive patients with APE was therefore shown to be a useful means to stratify these patients by risk. The authors concluded that these patients represent a group that may benefit from more aggressive therapies.

This important study is the longest published follow-up of patients with PE evaluated for echocardiographic evidence of persistent pulmonary hypertension and survival. PE patients were found to be more likely to have persistent pulmonary hypertension if their initial pulmonary systolic pressures were more than 50 mmHg or if they were older than 70 years. Those who had persistent pulmonary hypertension and RVD at 6 weeks were also less likely to survive at 5 years' follow-up.
