Hsu C, Cummings SR, McCulloch CE, Chertow GM. Bone mineral density is not diminished by mild to moderate chronic renal insufficiency. Kidney Int 2002; 61:1814-1820. This controversial epidemiological study challenges the concept that bone mineral density is affected early in renal failure. The study suggests that even though subjects with worse renal function have lower BMD, this association is explained by confounding, principally by sex, age and weight. Thus, renal function itself does not seem to be independently associated with BMD.

The Women's Health Initiative study focused on defining the risks and benefits of combined estrogen plus progestin in postmenopausal women with uterus. The astonishing and still controversial results of this study, which included over 160 000 women, have changed dramatically the decision-making process regarding HRT prescription in postmenopausal women. The results indicate that HRT should not be initiated or continued for primary prevention therapy of coronary heart disease.

This was a well designed pharmacological study that evaluated the dialysis removal of ibandronate, a potent bisphosphonate. The drug was efficiently removed by hemodialysis.

The study demonstrated that skeletal deposition of the bisphosphonate clodronate is related to bone turnover and that significant binding of the drug occurs in cases of low bone turnover. This should caution the practicing physician on the prescription of these drugs in ESRD.
