5. The problem of residual strain at the crystal surface is always a concern. In the present case, the conductivity is not large; as a consequence, the optical penetration depth is large, so that any surface strain should not significantly affect our bulk-sensitive measurement.

7. 4+δ, which are independent of x, will not affect significantly the conductivity, and in particular the spectral shape, in the measured frequency region.
8. Following common practice of the optical community, we present the frequency in terms of wave number v̄, where v̄= v/c = ω(2πc), with c being the speed of light.
