Transgenic mice engineered for glutamatergic excitation of orbitofrontal, limbic and efferent striatal circuitry exhibited OCD-like behaviours and juvenile-onset tics, which responded to clonidine. These data suggest a shared cortical-limbic glutamatergic basis for OCD and tics.

    • Leentjens AFG, Lousberg R, Verhey FRJ. Markers for depression in Parkinson's disease. Acta Psychiatr Scand 2002; 106:196-201. Logistic regression was performed on data from 161 patients with Parkinson's disease, 40 of which also had depression, to explore general and disease-specific risk factors. General risk factors predicted depression, particularly a family history of depression. Right-side onset was illness-variable only to improve the fit of the model.

    • Schuurman AG, van den Akker M, Ensinck KTJL, et al. Increased risk of Parkinson's disease after depression: a retrospective cohort study. Neurology 2002; 58:1501-1504. All subjects over age 20 diagnosed with depression from 1975-1990 in South Netherlands were identified from a general practice database and followed through to 2000; they were matched with a reference group of the same age but who had never been depressed. The hazard ratio for developing Parkinson's disease after depression was 3.13. This study, though retrospective and based on non-expert classifications, provides clear evidence for the prodromal presence of depression in Parkinson's disease.

A structured questionnaire was given to 50 patients with fluctuating motor symptoms of Parkinson's disease to monitor for cognitive, psychiatric and dysautonomic symptoms. All reported non-motor fluctuations in association with motor fluctuations, with anxiety (66%), drenching sweats (64%), slowness of thinking (58%) and fatigue (56%) being very common; 28% reported that the non-motor fluctuations were more disabling than the motor fluctuations.
