Diagnostic utility of pleural fluid eosinophilia

Adelman M, Albelda SM, Gottlieb J, et al.: Diagnostic utility of pleural fluid eosinophilia. Am J Med 1984, 77:915-920. A pivotal review of EPEs. The authors summarized the etiologies of 343 cases of EPE, reported in the English literature between the early 1960s and 1984. They also performed Bayesian analysis using the data from previously reported series with pleural effusions and concluded that the presence of pleural fluid eosinophilia decreases the probability of malignant and tuberculous etiology. On the other hand it increases the probability of a pleural effusion to be asbestos-related or idiopathic.

The authors found that malignancy is as prevalent among EPEs as non-EPEs, and that patients with EPE had better a prognosis than those with non-EPE regardless of the underlying disease.

An informative review in which the clinical, radiographic, and pleural fluid features in patients with pleural effusions resulting from pulmonary emboli are discussed. The author suggests that pulmonary emboli are a common cause of pleural effusion in the general population, and discusses the use of D-dimers, spiral CT, and duplex ultrasonography of the leg in the diagnostic approach of patients with pleural effusions and suspected pulmonary embolism.
