This Swedish study of 60 patients with IBD (49 CUC, 11 CD) and low-grade dysplasia appears to question the current recommendation of immediate colectomy when low-grade dysplasia is detected in flat colonic mucosa and confirmed by a second expert pathologist, because no cases of progression to high-grade dysplasia was evident in this study.

    • The fate of low grade dysplasia in ulcerative colitis
    • Ullman TA, Loftus EV, Kakar S, et al.: The fate of low grade dysplasia in ulcerative colitis. Am J Gastroenterol 2002, 97:922-927. This study of 18 patients with UC and low-grade dysplasia who were observed for a median of 32 months at a North American tertiary care center reported a cumulative incidence of 33% for progression to an advanced lesion at 5 years. This concurs with the current recommendation that total proctocolectomy should be offered to all patients with low-grade dysplasia in flat colonic mucosa. This study contradicts the findings of the study by Befrits et al. [2•].

A Swedish study of 604 patients with PSC followed up for a median of 5.7 years showed a 13.3% frequency of hepatobiliary malignancies, which was 161 times that of the risk in the general population. The PSC cohort also had a 10-fold increased risk of CRC and a 14-fold increased risk of pancreatic cancer in comparison with the Swedish general population. This increased risk of pancreatic cancer is a novel finding.

A report on 18 small duct patients with PSC, a largely unstudied patient group, revealed a comparable survival data in comparison with the general population of the United States. Some patients with small duct PSC appear to progress to classic PSC. However, hepatobiliary malignancies did not develop in any of the patients with small duct PSC.

A report on 14 of 102 patients with PSC who underwent cholecystectomy who had a gallbladder mass; 57% of the masses were malignant. Among the patients with benign masses, a third had associated epithelial cell dysplasia, whereas 57% of patients with primary gallbladder cancers had associated epithelial cell dysplasia. This suggests a dysplasia-carcinoma sequence. The authors recommend that cholecystectomy should be performed in all PSC patients with gallbladder polyps.

    • ARF gene in ulcerative colitis-associated colorectal carcinogenesis
