Hickie I, Davenport T, Issakidis C, Andrews G. Neurasthenia: prevalence, disability and health care characteristics in the Australian community. Br J Psychiatry 2002; 181:56-61. This recent community study from Australia confirms the high burden placed on primary health care by fatigue and its related disorders. Even after allowing for the expected oomorbidity and prediction of association, people with neuroasthenia also reported more affective, anxiety, and physical disorders.

Bultmann U, Kant I, Kasl SV, et al. Fatigue and psychological distress in the working population. Psychometrics, prevalence, and correlates. J Psychosom Res 2002; 52:445-452. Bultmann et al. looked at a cohort of Dutch employees of working age (n = 11859) and unearthed a prevalence of 22% for fatigue and 23% for psychological distress with a strong association between the above variables. Amongst those reporting fatigue, 57% of these also reported psychological distress. These patients also repoded more physical disease when asked to self-report. This probably is just further evidence that being ill (or believing yourself to be ill) is depressing and tiring.

Taylor RR, Jason LA, Curie CJ. Prognosis of chronic fatigue in a community-based sample. Psychosom Med 2002; 64:319-327. This community retrospective cohort study followed up patients with CFS. Perhaps rather limited by their small sample size (224/18 675) and only a 50% take-up rate at 2-year follow-up may have introduced bias (with healthy controls less interested in talking to an interviewer about fatigue).

Addington AM, Gallo JJ, Ford DE, Eaton WW. Epidemiology of unexplained fatigue and major depression in the community: the Baltimore ECA follow-up, 1981-1994. Psychol Med 2001; 31:1037-1044. This was a prospective population based study with 73% of the original group interviewed (after 17 years). Dysphoric episodes and number of somatoform symptoms were the only independent predictors of a new onset of unexplained fatigue. As they rightly comment in their discussion, temporality and causality cannot be commented on as both conditions occurred within the follow-up period.

This study examined 69 monozygotic and 31 dizygotic all female twin pairs assessed by a combination of lay researchers, and self-report of physical conditions. Fifty-five percent of the sample were picked by CFS newsletter adverts, introducing a potential source of bias.

Gagnon LM, Patten SB. Major depression and its association with long-term medical conditions. Can J Psychiatry 2002; 47:149-152. Gagnon randomly selected 2542 residents and via telephone interview asked about depressive symptoms and self report of chronic illness. Reports of having one or more medical conditions were associated with an increased risk of depression even with the confounders of increased age, sex, social support and recent life events removed. The strongest associations were found with vascular disorders, and brain diseases. This study was rather limited, however, by the abbreviated instrument they used and the reliance on self report of chronic disease. It seems likely that concurrent depression may well influence an individual's subjective report of physical illness.

Kurtze N, Svebak S. Fatigue and patterns of pain in fibromyalgia: correlations with anxiety, depression and co-morbidity in a female county sample. Br J Med Psychol 2001; 74 (Pt 4):523-537. Their large community sample in Norway found a prevalence of 3.2% of fibromyalgia with a sex ratio of 5:1 (female:male). They found partial correlation of anxiety and depression with fatigue and pain but when the comorbidity of an additional chronic disease was added in the relationship became more specific; fatigue being associated with depression, and pain with anxiety.

This is a systematic review of 44 studies (including 36 randomized controlled trials) of various treatment modalities. Each study was also assessed for 'validity' by two independent reviewers. Studies on CBT and graded exercise therapy were the only ones to score highly on both validity and efficacy.

Richards and Scott prescribed graded aerobic exercise to 132 patients with fibromyalgia from their rheumatology outpatients clinic, using relaxation as their control. Thirty-five percent of patients (control 18%) rated themselves as better or much better at 3 months with benefits maintained or even improved at 1-year follow-up. Patients also had greater reduction in the number of tender points, a benefit which lasted 12 months or more.

The authors comment that although any patients with a current diagnosis of major depression were excluded, most had a history of major depressive disorder, lending further weight to the idea that depression and fibromyalgia share common pathophysiology
