This highly cited paper is among the most important in the stress echo literature. It emphasises the need for a learning curve even among expert echocardiographers.

The first of a series of multicentre studies that have highlighted the consequences of subjective reading on the concordance of stress echo interpretation between experts. Although subsequent papers have chronicled an improvement with standard reading criteria and technical advances, the need remains for a standard quantitative tool.

J Am Coll Cardiol 1996;27:910-48. Probably the greatest growth area for stress echo over 5-10 years has been as a tool for risk stratification of patients undergoing major non-cardiac surgery. These guidelines highlight the importance of intelligent selection of patients, based on the nature of their planned surgery and their cardiac history.
