In situ hybridization was performed in parathyroid glands of renal patients operated for refractory HPP to investigate the expression of calcium receptor (CaR), calcium sensing receptor (CAS), vitamin D receptor (VDR), and PTH mRNAs in these specimen. Results confirmed decreased expression of CaR, CAS, and VDR compared with normal glands. PTH mRNA was also decreased, suggesting that the great increase in the number of parathyroid cells, and not increased synthesis per cell, was responsible for the high levels of serum PTH in renal HPP. Interestingly, the expression of the four investigated genes was highly variable between different glands as well as within individual glands. Each nodule appeared to have its own profile of mRNA expression, suggesting that the rapid cell proliferation of parathyroid cells increased the risk of mutation, leading to the formation of monoclonal nodules, the clonal origin of each nodule being separate.

This interesting study demonstrates that the "intact" PTH assay routinely used for measurement of serum PTH levels also reacts with 7-84 PTH fragment. This fragment is produced by uremic parathyroid gland and has an inhibitory effect on PTH activity. Thus, the "intact" PTH assay overestimates serum 1-84 PTH level and activity.

This electron-beam computed tomography study did not detect significant cardiac calcification in 23 dialysis patients aged less than 20 years but demonstrated that 14 of 16 examined dialysis patients aged 20 to 30 years had coronary artery calcification, at an age at which coronary calcification was seldom observed in controls. Coronary artery calcification was associated with high phosphatemia, serum calcium × phosphorus product, and calcium intake. Rapid progression of calcification was demonstrated in 10 patients examined after a mean interval of 20 months and appeared associated with the same factors.

Immunohistology demonstrates staining of calcified skin arterioles for osteopontin, suggesting that an active mechanism, which could involve local dedifferentiation of smooth muscle cells into osteoblast-like cells, is involved in media calcification.

This Japanese study reports on a series of 1053 total PTx with forearm autograft in renal patients. Despite an impressive attempt to remove ectopic glands by intensive preoperative exploration and peroperative excisions, 1.5% of patients experienced recurrence due to supernumerary gland. Graft recurrence rate increased with follow-up to 8.1%, but patients were easily treated as no muscle infiltration was observed at the forearm. Graft recurrence was mainly observed after the nodular parathyroid gland was transplanted and should be limited by avoiding nodular gland transplantation after macroscopic and intraoperative histologic examination.
