Late changes (umbilical artery reversed flow, abnormal ductus venosus, abnormal aortic and pulmonary outflow tracts) were the best predictors of perinatal death. This and the two previous studies represent an advance in our understanding of the vascular changes in the severely IUGR fetus.

The authors report that growth restriction is associated with abnormal ventriculovascular physiology. This represents a successful adaptive response to raised placental impedance and reduction in wall stress. However, whilst fetal adaptive mechanisms may aid survival, they may result in cerebral and vascular abnormalities that prejudice subsequent cardiovascular health.

The sensitivity of a pulsatility index above 2.35 for pre-eclampsia was 27%, whereas it was 11.7% for intrauterine growth restriction alone.

The uterine arteries were analyzed by using a score that varied from 0 (normal findings in both uterine arteries) to 4 (notch and high pulsatility index in both uterine arteries). At a uterine artey score above 2, there was a significantly increased risk of pregnancy complications. The last four studies are interesting. However, they do not solve the question of which index is the best and do not tell us when we need to evaluate the uterine arteries.

The cut-off point between severe anemia and non-anemia is higher that that for fetuses that have never been transfused, because of the different rheological properties of the adult blood used in intrauterine transfusion.

Br J Obstet Gynaecol 2002; 109:746-752. This represents the confirmation that the MCA PSV in the hands of trained operators may be used to diagnose anemia. This was a prospective study in which the MCA PSV was used for timing an invasive procedure (amniocentesis/cordocentesis) in fetuses at risk for anemia because of maternal red cell alloimmunization. Invasive procedures were avoided in 70% of the patients. Based on these data, the authors recommend weekly assessment of the MCA until 35 weeks of gestation.
