    • Pharyngeal pH measurements in patients with respiratory symptoms before and during proton pump inhibitor therapy

The authors document the efficacy of double, standard dose proton pump inhibitors in the reduction of pharyngeal reflux with pH monitoring after 3 months of therapy. Symptoms resolved in 89% (8 of 9) of patients with suppressed pharyngeal reflux. Four patients (75%) without pharyngeal reflux on pH monitoring were treated in an identical fashion because of their symptoms and results of the laryngeal examination. Three of four (75%) had persistent symptoms despite the same therapy.

The authors note that some reflux does occur in control subjects with normal examination findings, that symptoms alone are unreliable, and that physical findings do not predict abnormal pH study results. The importance of pharyngeal pH monitoring is discussed.

Interestingly, many of the "classic" symptoms of EER improved in both groups, indicating a significant placebo effect. Alternative explanations for the findings provide an excellent outline of the inherent limitations in the study of EER.

Symptoms improved over the first 2 months of therapy with proton pump inhibitors while the changes in physical findings continue to 6 months, and perhaps beyond 6 months. The authors conclude that proton pump inhibitor therapy should continue for a minimum of 6 months while laryngeal inflammation subsides.

The reliability of the video laryngeal examination in the diagnosis of EER is evaluated. The authors note significant inter-rater and intrarater variability in the analysis of video laryngeal examination results in suspected EER patients. They note that despite variability for individual signs, the raters were consistent with respect to the complete examination and to which patients warranted treatment.
