A representative survey (response rate > 70%) containing the most recent data in the field. It showed a high awareness of AAS prophylaxis also in non-obstetric patients and before regional anaesthesia; metoclopramide was used rather frequently. There was also a relaxed approach towards fasting, with clear liquids being allowed in adults up to 2 h before the procedure by approximately 50% of respondents.

Excerpts from the Australian Incident Monitoring Study, comprising 226 incidents (4%) in obstetric anaesthesia, among which only one aspiration led to AAS. Very interesting reading despite possible bias due to voluntary reporting. An increased number of incidents in obstetrics was reported in the sub-sections of regional anaesthesia, drug incidents, and failed intubation compared with non-obstetric cases.

•], but evaluating all aspiration incidents. It is worth noting that emergency, inadequate anaesthesia, and obesity were among the leading risk factors for regurgitation, vomiting, and aspiration. Of 131 incidents of aspiration, 118 (90%) occurred while a laryngeal or face mask was being used.

The delayed publication of data collected in 1993 with sufficient recovery (33% of institutions covering 61% of births in the USA). Comprises answers from anaesthetists and obstetricians. Policies are mainly limited or unlimited clear liquids in the early stage, limited clear fluids or nil by mouth in the late stage of labour. Unfortunately, the authors do not compare their data with a similar set from 1986, and they fail to indicate differences between obstetricians and anaesthetists (or compare the answers of the two specialists working in the same hospital).

Otherwise a must for every anaesthetist working in the field of obstetrics; however, the Task Force is not commenting on aspiration prophylaxis other than in terms of fasting guidelines. The experts agree that 'regional anesthesia (for cesarean section) can be administered with fewer maternal and neonatal complications when compared to general anesthesia' but recommend that 'the decision should be individualized based on anesthetic, obstetric, and/or fetal risk factors and the preferences of the patient and anesthesiologist'.
