This article reviews the traditional use of dobutamine, dopamine, and intravenous milrinone for decompensated advanced heart failure and provides an objective attitude toward the controversial "intermittent infusion" approach. Based mainly on the authors' own findings, current applications are critically debated and recommendations are given, including those for the nontransplantable heart failure patient.

This is a critical comment on the current use of inotropic infusions for the management of both inpatient and outpatient chronic heart failure. The author presents the view that this therapy should only be recommended as temporary support for stunned or hibernating myocardium that has been revascularized, for an acutely ill patient until it is determined that there is a treatable or reversible cause, or as a bridge to cardiac transplantation.
