Although DSM-IV guidelines currently recommend excluding Axis II symptoms that are accounted for by an Axis I disorder, the majority of studies ignore this recommendation. This study is the first both to attempt to distinguish between personality disorder symptoms that are independent versus substance-related as well as to compare independent and substance-related diagnoses with respect to reliability and clinical correlates. Importantly, the findings do not clearly support the validity of this exclusion strategy, thereby raising questions about the validity or feasibility of the recommendations in DSM-IV.

This study examines the associations between personality disorders and different types of childhood maltreatment in 378 adult substance abusers. The methodology (path analysis) is innovative. The results indicate that childhood emotional abuse predicts later personality disorders, except for antisocial personality disorder (differentially predicted by physical abuse and physical neglect) and schizoid personality disorder (differentially predicted by emotional neglect).

This up-to-date, innovative and comprehensive review provides a heuristic framework that might stimulate further human and animal experimental research by generating clear hypotheses with respect to the relationship between personality traits (or behavioural tendencies in animals) and dysregulations in several distinct neural circuitries or neurotransmitter systems on the one hand, and physiological and subjective cue reactivity (craving) and responses to anti-craving agents on the other hand.

This original work contributes to an understanding of the personality dimensions that are specifically associated with different psychoactive substances.

    • 55 Project Match Research Group. Project MATCH secondary a priori hypotheses. Addiction 1998; 92:1671-1698. This study reports on empirical data from the largest and most rigorously designed matching study ever conducted. Three protocollized psychosocial treatments (cognitive-behavioural coping skills training, motivation enhancement therapy, and 12-step facilitation) are compared and 10 different matching variables (among which sociopathy as a personality variable) are examined. The finding that outpatients high on psychiatric severity benefit more from 12-step facilitation than from cognitive-behavioural training is the only robust matching effect reported.
