Transplant Proc 1999, 31:2537-2538. Survival rates in a series of 26 patients age 65 and older transplanted between 1991 and 1998 are compared with survival rates of 194 patients under the age of 65. One year survival was 76.3% in the older and 77.6% in the younger patients.

Overall 1-and 5-year survival rates were 87.8% and 73.4%, respectively. Contrary to multicenter registry reports, increasing donor age, recipient age, and ischemia time up to 4.2 hours were not risk factors for mortality after heart transplant.

Increased mortality in the older patients was seen, with the hazard being highest in the first year, and most commonly due to infection.

J Heart Lung Transplant 1998, 17:901-905. This study assessed the impact of donor heart age on recipient morality in patients who were outpatients at time of transplantation, using registry data. The risk of death due to complications of receiving an older donor were compared with the risk of death while waiting for transplantation. These results suggest that the use of older donor hearts, despite increased risks in relation to the use of younger donor hearts, improve patients' survival compared with not receiving transplantation.
