Echocardiographic prediction of survival after surgical correction of organic mitral regurgitation

A follow-up study of 409 patients operated on for treating pure organic mitral regurgitation. In multivariate analysis, echocardiographically determined ejection fraction was the most powerful predictor of long duration of survival, even for patients who had valves repaired. Results of this large study suggested that, even for patients with severe mitral regurgitation without severe symptoms, early surgery should be considered in order to preserve left ventricular function. This appears feasible since operative mortality was low (1.1%) for patients aged less than 75 years.

The incidence, prognosis, and predictability of left ventricular dysfunction after operation for 266 patients undergoing correction of mitral regurgitation were examined. Operation decreased left ventricular ejection fraction significantly, from 58 to 50%. Interestingly, the mean reduction in ejection fraction was greater for patients who had valves repaired. Left ventricular dysfunction (defined as ejection fraction < 50%) after operation was common (occurring for 41% of patients) and was attended by a poor prognosis.

Clinical follow-up of 229 patients with isolated mitral regurgitation due to flailing leaflets diagnosed by echocardiography revealed excess mortality and high morbidity among patients who had been treated medically. Independent determinants of mortality were greater age, the presence of symptoms, and lower ejection fraction. Among patients who were even transiently in functional class III or IV there was a high mortality rate (34% yearly), but the rate among those in classes I and II was also notable (4.1% yearly). In multivariate analysis, surgical correction of mitral regurgitation (performed for 143 patients) was associated with longer duration of survival. The homogeneous nature of this cohort of mostly asymptomatic and mildly symptomatic patients clarifies the 'natural history' of severe mitral regurgitation in an era of medical and surgical treatment options.

Results of this study confirm the superiority of repair over replacement of valves for improving clinical outcomes. Although there were significant baseline differences between the 195 patients who had conservative surgery and the 214 whose valves were replaced, multivariate analysis indicated that repair had an independent beneficial effect on overall duration of survival, operative mortality, and ejection fraction after operation.

Actuarial survival estimates indicated that surgery conferred an advantage but there were significant baseline differences between the groups. Notable amongst the 23 deaths among medically treated patients of which the cause was determined were 11 that occurred suddenly.

Of these 15, four eventually required surgery after a short mean follow-up period of 15 months on the basis of progression of symptoms. The authors recommend frequent clinical and echocardiographic surveillance since progression of left ventricular and left atrial dilatation in their patients was not a function of time.

Independent predictors of survival for patients in the medical cohort included left ventricular end-diastolic volume, whereas no variables other than age were predictive after surgery. Survival curves indicated that surgery was of benefit only for patients with 'moderate impairment' of left ventricular systolic function (ejection fractions 31-50%).

These authors described the natural history of 31 asymptomatic and minimally symptomatic patients with severe nonischemic mitral regurgitation caused by prolapse of mitral valves, who had normal right and left ventricular function. There was a 10% annual risk of progressing to significant symptoms or 'high-risk' ventricular performance descriptors. Fourteen patients developed symptoms warranting referral for operation; none had 'high-risk' ventricular descriptors but the thresholds for abnormality in this study were unusually low (e.g. left ventricular ejection fraction < 45%).

Circulation 1997; 96:1819-1825. The outcomes of 63 patients who had had early mitral valve surgery (within 1 month of diagnosis) were compared with those of 158 patients initially treated conservatively (80 of whom were operated on later). The early-surgery strategy led to a better overall survival rate and a lower cardiovascular morbidity (cardiovascular deaths, CHF, and new chronic atrial fibrillation), with an independent benefit that was confirmed by multivariate analysis. Authors of this study were the first to demonstrate that there is an advantage in terms of outcome from early correction of mitral regurgitation.

Measurements of elastance of left ventricle chamber were consistent with an abnormality of contractile function of patients with long-standing mitral regurgitation and normal ejection fractions. In some cases, the abnormality was reversible after replacement of mitral valves. The practical importance of these data is that, because early contractile dysfunction in mitral regurgitation is difficult to detect and often occurs in chronic mitral regurgitation, a strategy of valve surgery early rather than later, in order to preserve contractile function of these patients, is justifiable.
