Neobladder patients adapt better to the new situation than conduit patients.

This fascinating article suggests that those men who are continent after orthotopic neobladder have a lower sensory threshold at the membranous urethra than those who are incontinent. This suggests that continence may to a large degree depend on the pelvic nerve supply to the sphincter mechanism, and that there may be a maximum achievable result with modern surgical techniques.

Urology 1999; 54:252-257. This article presents the case against the frequently heard statement that all reservoirs offer approximately the same. The truth is that there are huge differences! The pressure within the reservoir is inversely related to its radius. The same group of surgeons compared the urodynamic parameters and continence rates among the five different urinary reservoirs they constructed. The results produced underline the laws of physics: p=T/r (La Place's law; where 'p' is pressure, 'T' is flow and 'r' is radius).

J Urol 1999; 161:422-428. The long-term complications and functional results in 363 ileal neobladders with a mean follow up of 57 months are presented. This study demonstrates the safety and efficacy of ileal neobladder urinary diversion in experienced hands. The study had the longest follow up and largest series; the complication rates were acceptable (but not negligible) functional results were excellent.

This article analyses the functional results of orthotopic bladder substitution. Six out of 43 women had difficulty emptying the pouch spontaneously, with concomitant residual urine due to acute angulation between urethra and pouch. Failure to complete emptying seems to be due to anatomical rather than functional reasons.
