Understanding the anatomic variations in radiography of the cervical spine in small infants and children can help avoid overdiagnosis. Magnetic resonance imaging can show anatomically exquisite portrayals of growing skull fractures for neurosurgical planning, but its routine use for imaging the cervical cord in shaken, abused infants without clinical abnormality appears to be unwarranted.

Lateral radiographs had a sensitivity for cervical spine injury of 79%; a three-view radiographic series had a sensitivity of 94%. All patients with cervical spine injury who were clinically asymptomatic had both a high risk injury mechanism and a distracting injury. Cervical spine injury should be suspected in any child with abnormal findings on the neck or discovered in a neurological examination.

This study demonstrates that children at 3 years of age and older who sustain a type II odontoid fracture with an intact transverse ligament can be safely managed with odontoid screws if the fracture is less than 4 weeks old. Posterior C1-C2 transarticular screw fixation can be done safely and results in a high fusion rate in children older than 4 years of age. The technical difficulties of screw fixation in children also are discussed.

7 Logan PM: Is there an association between fractures of the cervical spine and first and second rib fractures? Can Assoc Radiol J 1999, 50:41-43. Almost one third of patients with traumatic cervical spine injury also have an associated upper rib fracture. The strongest association is between injury at the C7 level and the first rib fracture.
