In an attempt to learn the natural history of glenohumeral deformity in perinatal brachial plexus palsy, the authors qualify glenohumeral deformities using radiographic (CT or MRI) representation of the patient's anatomy. A graded scale of deformity was then employed to distinguish patients who required rotational osteotomy rather than contracture release and tendon transfer.

In addition, the author found equivalence in Mallet Scores (at two years of life) between the microsurgical outcomes of his patients without biceps function at 6 months to conservative outcomes of his patients who developed biceps function between 3-6 months.

The article analyzes the results of a new technique for muscle transposition in shoulder deformities caused by brachial plexus injuries. The surgical technique takes advantage of co-contraction of synergists and antagonists. The contracted, antagonistic muscles are transferred on their neurovascular pedicles to sites where they can function as synergists with paretic muscles. Average shoulder abduction increased 104% and external rotation 200%, and adduction strength decreased.
