The authors report the cases of six infants with chylothorax of congenital or postoperative origin. They demonstrate that although the morphologic features of the atypical mononuclear cells found in such effusion are worrisome, the use of flow cytometric immunophenotypic analysis is helpful in identifying these cells as benign T cells of peripheral lymphatic origin.

Chylothorax was observed in 29% of patients during the overall course. Patients with chylothorax were older than patients without chylothorax. Chylous ascites was observed in 12% of patients. Lung transplantation was performed in 13 patients, without recurrence of lymphangioleiomyomatosis in allograft after 2.3±2.2 years.

Seven patients with refractory chylothorax, four congenital and three iatrogenic, were treated for an average of 44 days with a pleuroperitoneal shunt. The shunt survival rate was six of seven (86%) and patient survival rate was five of seven (71%). The authors conclude that pleuroperitoneal shunting is a safe, simple, and effective treatment of chylothorax in infants.
