Br J Ophthalmol 1998, 82:661-665. This meta-analysis on recurrence rates after various surgical procedures for primary pterygium proves conclusively that bare sclera resection is inferior to other forms of pterygium surgery and should no longer be done routinely. However, the study shows no significant differences in recurrence rates with conjunctival autografting and bare sclera excision combined with mitomycin C. Limitations of this study include the paucity of randomized, controlled, clinical trials and the fact that appropriate studies on β-irradiation were not available for comparison.

This study evaluated a modification of conventional conjunctival autografting in which the original epithelium from the pterygium is re-applied after a 180° rotation. Conjunctival rotation autografting does not require the harvesting of superior bulbar conjunctiva, and it is therefore a useful alternative in cases in which conventional autografting is contraindicated or difficult.

Dua shows that in the presence of partial limbal stem cell deficiency, the careful, repeated removal of conjunctival overgrowth onto the cornea preferentially allows corneal re-epithelization. Dua suggests that this replacement of conjunctival epithelium with corneal epithelial cells may be important in explaining the phenomenon of conjunctival differentiation and is a therapeutic option in eyes with partial limbal stem cell deficiency.

Previous studies confirmed the usefulness of amniotic membrane in ocular surface transplantation, and Tseng et al. now show that AMT alone may be sufficient to treat eyes with partial limbal deficiency with superficial involvement. It is postulated that reduction of ocular surface inflammation and vascularization by the presence of amniotic membrane is responsible for stabilization of the ocular surface in these eyes.

This suggests that amniotic membrane may be an ideal replacement for conjunctiva because the two tissues share basement membrane properties.
