In this study, the variable clinical presentation among patients with Pendred syndrome is highlighted by comparing 18 index cases from 18 kindreds with 18 siblings, all with positive linkage to the PDS locus. Although some parameters are not presented in detail, the study indicates that the use of clinical criteria alone will result in underascertainment of Pendred syndrome.

The boy presented in this case report developed rapidly progressive sensorineural hearing loss during the first years of life, indicating that the deafness is not congenital in alt patients with PDS mutations. By palpation he did not have a goiter, but his thyroglobulin levels were elevated. Thyroid hormone levels were within the normal range. On computer tomography, he had a displastic cochlea and an enlarged vestibular aqueduct. Molecular analysis of the PDS gene revealed compound heterozygosity for PDS mutations.

Radiologic evaluation of the inner ear documented a large vestibular aqueduct in association with an enlargement of the endolymphatic sac and duct in all patients with Pendred syndrome studied by MRI. The results suggest that malformations of the membranous labyrinth are a more specific diagnostic sign for Pendred syndrome. Together with the studies of Li [36·], Usami [37·], and Abe [55·] that reported an enlarged vestibular aqueduct in all patients with the allelic variants of Pendred syndrome, this study suggests that an enlarged vestibular aqueduct may be a consistent finding in disorders caused by mutations in the PDS gene.

J Clin Endocrinol Metab 1999, 84:336-341. In this highly inbred kindred with multiple individuals with deafness and goiter, it was impossible to establish linkage to chromosome 7q31. Mutational analysis of the PDS gene revealed that some individuals have true Pendred syndrome, while others have goiters or deafness caused by distinct environmental or genetic factors. In this study, the problem of phenocopies in linkage studies is illustrated, emphasizing that the diagnosis of Pendred syndrome may be difficult if it has to rely primarily on clinical findings and underscoring the value of molecular diagnosis.

In this kindred with nonsyndromic autosomal recessive deafness, linkage could be established to chromosome 7q31. Goiters were absent and enlarged vestibular aqueducts were found in the 3 affected individuals that could be evaluated by computer tomography. Mutational analysis of the PDS gene revealed homozygosity for 2 substitutions (G97S, 1490L) in affected individuals. This supports the notion that the nonsyndromic deafness found to be linked to the DFNB4 locus on chromosome 7q31 is an allelic form of the syndromic hearing loss in Pendred syndrome.

Three families were found to be homozygous, and 3 families were compound heterozygous. None of the patients fulfilled the criteria for Pendred syndrome. The study defines hearing loss associated with nonsyndromic familial enlarged vestibular aqueduct as an allelic variant of Pendred syndrome.

In contrast to the sodium-iodide symporter, pendrin does not require the presence of sodium. Pendrin-mediated anion-transport can be partially inhibited by nonspecific transport inhibitors.
