This is the first study in a larger consecutive (n = 20) series followed up for 12 months, starting with the very first patient treated by bilateral stimulation of the STN. The study confirms the preliminary reports from this group that demonstrate a major improvement in off-period motor score (UPDRS 111,60%), painful off-period dystonia (resolved in 75%, decreased in the remaining patients) and global activities of daily living (Schwab and England scale increased from 29% to 74% on average). In addition, dopaminergic drugs could be reduced by half with a subsequent reduction in dyskinesias. Five patients were followed for more than 36 months, and the treatment had a continuing benefit in all of them. The therapy can be considered as a breakthrough for Parkinson's disease patients with advanced stage disease, suffering from severe long-term motor complications of dopaminergic treatment.

In contrast to the Grenoble experience, the Toronto group found a significant improvement in on-period motor function. Unlike in the study by Limousin et al., however, the patients were not assessed with suprathreshold levodopa dosage, which may account for this difference.

Pallidotomy had no effect on initiation time in unwarned simple and choice reaction time tasks, whereas movement time measured during the same task was improved. Movement times also improved for isometric and ballistic movements. In contrast, repetitive, distal and fine movements measured in finger-tapping and pegboard tasks were not improved after pallidotomy. Preparatory processes were investigated using both behavioural and electrophysiological measures. The data suggest that pallidotomy improved mainly the later stages of movement preparation and the execution of proximal movements with the contralesional limb. This paper provides detailed quantitative data on the impact of PVP on upper limb akinesia.

Dyskinesias were suppressed, together with rigidity, when stimulating the ventral GPi or the ansa lenticularis. Stimulation effective for these symptoms could worsen akinesia. On the other hand, akinesia was improved with stimulation of electrodes that were located more dorsally, sometimes with an increase in dyskinesias. It was possible to find a compromise between these opposite effects, with stimulation of an electrode located centrally in the GPi improving all of the symptoms. These findings confirm the results of earlier studies [77,78] and demonstrate that the pathophysiology of akinesia and rigidity is subserved by different anatomical structures within the GPi.

The effect of chronic high-frequency stimulation of the STN on diphasic and peak-dose dyskinesias is more complex and is related directly to the functional inhibition of the STN and indirectly to the replacement of the pulsatile dopaminergic stimulation by continuous functional inhibition of the STN. Fixed off-period dystonia, mobile diphasic dystonia and peak-dose choreodystonia seem to represent a continuous clinical spectrum of underlying activity patterns in STN neurones.

The effects of DBS of the STN and GPi on upper limb akinesia in Parkinson's disease were carefully examined. Stimulation improved movement time and force production to a larger extent than initiation time. Repetitive movements involving fine distal coordination showed only very modest improvements compared with proximal ballistic tasks. The STN group tended to show somewhat more benefit from stimulation on many of the measures, but the differences were minor, and were only quantitative, not qualitative. The effects of DBS of both targets were similar to the effects of unilateral pallidal lesions [40]. The authors suggested that disrupted pallidal outflow is the most important determinant of upper limb akinesia.
