Competency based instruction in critical invasive skills improves both resident performance and patient safety
-
Martin M, Vashisht B, Frezza E, Ferone T, Lopez B, Pahuja M, Spence RK. Competency based instruction in critical invasive skills improves both resident performance and patient safety. Surgery 1998; 124:313-317. A cadaver laboratory was used for the structured teaching of basic invasive techniques (endotracheal intubation, chest tube insertion and venous cut-down) to first year surgical residents. The increased skills were transferable to clinical settings.

The unreliability of physician report cards for assessing the costs and quality of care of a chronic disease
-
Hofer TP, Hayward RA, Greenfield S, Wagner EH, Kaplan S, Manning WG. The unreliability of physician report cards for assessing the costs and quality of care of a chronic disease. JAMA 1999; 281:2098-3004. Physician 'report cards' for diabetes, among the high prevalence diseases in medical practice, could not detect reliable practice differences between participating physicians. The system, allows the physician to eliminate data for difficult-to-control patients.

Assessment of clinical performance during simulated crises using both technical and behavioral ratings
-
Gaba DM, Howard SK, Flanigan B, Smith BE, Fish KJ, Botney R. Assessment of clinical performance during simulated crises using both technical and behavioral ratings. Anesthesiology 1998; 89:8-18. Video tapes were evaluated for two teams managing one of two simulated scenarios: Malignant hyperpyrexia or cardiac arrest. Both technical and behavioral performance could be assessed. Behavioral rating could be improved. A particular difficulty was the aggregation of single ratings for fluctuating behavior over time. The rating system needs more refinement before it can be used to assess global competence.

Testing internal consistency and construct validity during evaluation of performance in a patient simulator

Devitt JH, Kurrek MM, Cohen MM, Fish K, Noel AG, Szalai JP. Testing internal consistency and construct validity during evaluation of performance in a patient simulator. Anesth Analg 1998; 86:1160-1164. Using CPS, the reliability of anesthesia clinical performance was assessed. Six of 10 areas for assessment proved to be valid. Significantly, the study was able to discriminate between the performance levels of resident and faculty anesthesiologists.
