A complex algorithm for evaluation and testing of patients with repetitive arousals and sleep fragmentation is presented.

Compelling evidence that proactive application of ventilator support to asymptomatic patients does not prolong life is presented. This article could be the cornerstone for future research that will define standards for more exact timing in the institution of respiratory system maintenance.

57. Clinical indications for noninvasive positive pressure ventilation in chronic lung disease, COPD and nocturnal hypoventilation: a consensus conference report. Chest 1999, 116:521-534. A consensus statement for the use of noninvasive positive pressure ventilation (NIPPV) underscores the importance of performing multichannel sleep studies to detect associated sleep apnea in patients with neuromuscular disease and clinical course compatible with sleep apnea. Physiologic criteria for use of NIPPV are listed but represent only consensus. The participants clearly sight the need for quality randomized, controlled clinical trials to define further the physiologic indicators and proper timing of intervention with NIPPV.
