Neuron 1998, 20:483-494. Two studies [2••,3••] employing null mutants identify critical regulators of the development of mammalian cranial sensory neurons and propose models for their actions on the basis of genetic studies of Drosophila neuronal specification pathways. Both factors are basic helix-loop-helix transcription factors. Neurogenin 1 can induce the formation of neurons from ectoderm in Xenopus, and acts early to initiate a cascade of basic helix-loop-helix gene expression necessary for the formation of proximal cranial ganglia. Neurogenin II has a complementary function, specifying the epibranchial placode-derived cranial sensory neurons.

A new Enac-like two-transmembrane putative ion channel subunit named 'pickpocket' has been found to be present in the sensory dendrites of mechanoreceptive and proprioreceptive neurons. A further subunit, 'ripped pocket', generated functional sodium channels that were blocked by gadolinium. The authors propose that these proteins may represent functional mechanoreceptors loci in vivo.

Nature 1998, 392:394-397. Three complementary studies utilizing knockout mice [14•-16•] address the role of Neurokinins and their receptors in pain perception. All three support a role for substance P in nociceptive pathways but interesting discrepancies are apparent between the substance P/NKA null made with C57BI6 embryonic stem cells on a C57BI6 background, and the C57BI6/129 substance P/NKA null mouse. A dramatic loss of thermal sensitivity and formalin-induced hyperalgesia is noted by Zimmer et al. [14•] whereas a much more specific loss of sensitivity to moderately intense pain is observed by Cao et al. [15•]. Deletion of the NK1 receptor through which substance P acts by DeFelipe et al. [16•], who give no details of genetic background, appears to cause deficits in stress-induced analgesia and aggressive responses to territorial challenge in mice, as well as altered spinal cord processing of nociceptive signalling.

Nociceptin acts at the broadly expressed orphanin, ORF-1, receptor to cause hyperalgesia and allodynia (pain induced by normally innocuous stimuli) when administered intrathecally, although some other routes of administration suggest an opposite action of this peptide. The broad expression of ORF-1 has led to speculation that nociceptin may be involved in functions other than pain pathways. In this study, a biologically active peptide, named nocistatin, which derives from the nociceptin peptide precursor, has been shown to block the allodynic actions of nociceptin as well as pain induction by PGE2.

The novel sodium-channel-like transcript NaN was found in an overlapping subset of neurons to those that express the sodium channel SNS, and is also downregulated by axotomy, implying a dependence on axonally transported trophic factors. There are three fewer positive charges in the S4 voltage sensor domains of NaN compared to SNS. Despite the presence of voltage sensors, the inactivation tripeptide IFM and the sodium selectivity residues characteristic of voltage gated sodium channels, the channel shows <53% identity with other sodium channels.

Using antisense oligonucleotides that are effective in downregulating the functional expression of the SNS sodium channel in rat DRG neurons in vitro, the authors show that PGE2-induced hyperalgesia can be attenuated in the rat. Intrathecal administration of antisense but not sense or scrambled oligonucleotides were effective. In the absence of specific sodium channel antagonists, and with the usual caveats about the use of antisense strategies (specificity, toxicity etc. - caveats which apply equally to drug antagonists), this study is important in correlating the expression of a specific sodium channel with a behavioural effect - prostanoid induced hyper-reactivity in the flexion reflex.

A demonstration in a PET study of human subjects that affective reaction to painful stimulation is associated with activitiy of a particular part of the anterior cingulate gyrus. Some somatosensory cortical areas were not specially activated by the unpleasantness of a stimulus. Hypnosis was used to dissect the affective aspect.

PET is used to compare responses of human male and female subjects to warm and painful stimuli. Similar regions are activated in the two genders but there is quantitatively greater activation of the contralateral premotor cortex in females than in males.

AMPA/Kainate antagonist cause antinociception as well as ataxia. Selective antagonists of GluR5 and GluR2 also caused both antinociception and ataxia. However, an antagonist directed against GluR2 caused ataxia without nociceptive effects, whereas a GluR5-selective antagonist caused antinociception without ataxia. The experiments were repeated with enantiomers to demonstrate the stereoselectivity of the effect. These observations, taken together with the known high-level expression of GluR5 on primary sensory neurons, suggests that GlurR5 is an interesting analgesic drug target.

Using the polymer fucoidin which binds to L and P-selectins and blocks leucocyte accumulation at inflammatory sites, the authors demonstrate that leucocyte infiltration and beta-endorphin levels in inflamed tissues are reduced. At the same time, stress-induced and cortocotrophin releasing factor induced mechanical anlagesia are dramatically reduced. They suggest that new analgesic drug development strategies could target leucocyte migration to sites of tissue injury, although the signals that first induce leucocyte infiltraton via selectins remain to be defined.
