This study exemplifies the problem of the lack of a 'gold' standard, in that reliance on a DSM definition of depression may confine the sample to a subset of the depressed population. The authors elegantly describe the problems associated with defining and measuring depression in the medically ill. They compare symptoms of depression in different patient groups (psychiatric and chronically medically ill), Anhedonia, low positive affect and physiological hyperarousal were suggested as most discriminating of depression in the medically ill

The authors compare eight-item, four-item and one-item versions of the GDS and report that all give sufficiently high specificity and sensitivity to be used as an adequate screening instrument. The brevity of the one-item strategy may encourage its incorporation into standard use by nonpsychiatric staff

Pharmacotherapy is carefully addressed. The SSRIs may play a role in treating depression in this population while also reducing the risk of thrombosis

The nature of the association is unclear but may have a biochemical basis. TCAs are associated with worsening diabetes control because of their predominant influence on noradrenaline, whereas the SSRIs are associated with significant reductions in blood glucose levels and this is related to their predominant effect on serotonin over noradrenaline reuptake

However, lessening of depression scores on the Beck Depression Inventory was associated with a treatment-independent improvement of glycaemic control
