Br Med J 1998; 317:303-307. The methodologically most sophisticated study of CBT for psychotic symptoms demonstrates that one-third of schizophrenic patients experience a 'major' reduction in symptoms.

Three-year trials of personal therapy among schizophrenic patients living with or independent of family, I: Description of study and effects on relapse rates. Am J Psychiatry 1997; 154:1504-1513. Personal therapy is designed to buffer the individual from stress and to enhance coping strategies. This trial shows that relapse in patients living with a family is considerably reduced.

Relapse and rehospitalisation during maintenance treatment of schizophrenia: the effects of dose reduction and family treatment. Arch Gen Psychiatry 1997; 54:453-463. This multicentre study of family intervention fails to find a protective effect of intervention on relapse or interactions with dose reduction. An excellent discussion emphasizes constraints upon conclusions from these data.

Int Clin Psychopharmacol 1998; 13 (Suppl):S23-S30. A breakthrough study showing that high risk factors for psychosis when juxtaposed predict transition to psychosis in one half of the cases.

This study shows that when the grip is released following a successful (early) relapse prevention program, relapse proliferates. Continued input through the 'critical period' is necessary.

This theoretical paper suggests that stabilizing and controlling psychosis in the first 3 years is a critical period for long-term prevention. The previous article [21] supports this contention.

Psychol Med 1997; 27:799-806. Further evidence is presented supporting the excess incidence of psychosis among second generation Caribbean immigrants to the UK.

This study confirms the above results in London and reports a poor 1-year outcome for the black Caribbean group. Excellent discussion regarding the role of environmental factors.

Underdetection of severe morbidity at primary care was linked to ethnicity.

35. Close H, Garety P. Cognitive assessment of voices: further developments in understanding the emotional impact of voices. Br J Clin Psychol 1998; 37:173-188. This study finds that voice hearers' coping strategies (but not always their affect) are linked to their appraisal of the voices' purpose.
