Gastroenterology 1998, 114:724-736. Small-intestinal myoelectrical activity and intraluminal pressure changes in isolated intestinal tissue and in vivo movement of intraluminal contrast under fluoroscopy were studied in normal mice and in mutant W/Wv mice, which lack ICCs. Synchronized electrical and pressure waves and normal peristaltic activity were seen in control mice, whereas seemingly random electrical and pressure waves and nonpropagative motor activity were seen in mutant mice. The demonstration of abnormal in vivo motor function in addition to abnormal properties of isolated intestine in mutant mice strongly supports a central role for ICCs as physiologically important pacemaker cells.

Verapamil treatment following infection with Trichinella spiralis, a widely used model of intestinal inflammation, reduced the frequency of giant migrating contractions and diarrhea without affecting the inflammatory response. This study suggests that L-type calcium channels are involved in the induction of giant migrating contractions and diarrhea in the setting of T. spiralis-induced inflammation.

Small-bowel manometry studies span several hours at minimum, during which fasting and postprandial motility is recorded. In this study, findings during the first 3 hours of the fasting period and first 2 hours of the postprandial period of long recording sessions (at least 15 hours) agreed with the findings of the full study in 81 of 91 cases. This suggests that shorter recording sessions agree with longer sessions in the majority of cases, but the authors suggest monitoring at least 6 to 7 hours of fasting activity to improve diagnostic accuracy.

This series explores the symptoms and ultimate oncologic diagnoses of 162 patients with type 1 antineuronal nuclear antibodies, markers often associated with paraneoplastic syndromes. In 19 patients, the initial presentation was with pure gastrointestinal symptoms, serving as a reminder that occult malignancy may present as a dysmotility syndrome.

Great interest has been generated in the potential role of gut motor dysfunction in functional bowel diseases, but no discrete manometric patterns have been associated with illness or symptoms. In this study, ambulatory small bowel manometric studies revealed a high prevalence of dysmotility in 14 patients with motility-like dyspepsia, but no specific patterns discriminated patients from healthy subjects and no correlation was found between symptoms and motility patterns, indicating that the potential relationship between dysmotility and functional symptoms remains to be clarified.
