Patients with a residual mass smaller than 3 cm after chemotherapy do not appear to benefit from surgery. The authors of this report found that 30% of residual masses larger than 3 cm were residual tumours and that poorly defined masses could not be completely resected 56% of the time. The time from primary treatment to surgery was not reported which is a limitation of this study. Metastatic seminoma resolves slowly and early surgery reveals residual disease that may well be destined for resolution. Months may capse before a recognizable mass becomes thickening or even disappears.
