The principles of surgery for hypospadiac disease were utilized to advance the urethral meatus towards the lip of the glans.

Results of a large series of patients undergoing primary endoscopic urethral realignment, emphasizing that this does not have the same morbidity associated with open primary realignment and if the opportunity arises it may be of some benefit, particularly in bringing together the two ends of the urethra and thereby facilitating any subsequent surgery. In this study, only four patients subsequently underwent urethroplasty which is particularly impressive with the mean follow-up of 10.5 years.

J Urol 1998; 16:110-164. This paper describes a large series of boys with pelvic fracture urethral distraction defect (n = 30). This emphasizes the higher incidence of bladder neck injury in this age-group, with 15 patients undergoing an abdominal approach. The results were very satisfactory in that success rate of restoring urethral continuity is very similar to the previously reported adult series. Three patients had severe incontinence and in a further child, incontinence was related to overflow as a result of a poorly functioning detrusor. In the three patients with severe incontinence, incontinence caused by weakness of the outlet was related to the initial trauma.
