The authors investigated by metabolic labelling experiments an apo(a)-isoform intracellular retention of apo(a) that regulates apo(a) secretion. They further convincingly demonstrated by several means of blocking the secretory pathway that no intracellular assembly occurs, at least in this cell line.

The physiological significance of this finding remains to be clarified since an Lp(a) particle of such a small size does not exist in humans.

First in-vivo kinetic study of Lp(a) in humans (somehow cryptically hidden in the title) using the stable isotope technology. By this method, the authors were able to separately investigate the kinetics of the apo(a) and apoB-100 moieties of Lp(a). They found an almost identical synthesis rate for both proteins, which is compatible with intracellular Lp(a) assembly. Since these results definitely exclude the possibility of extracellular assembly of newly synthesized apo(a) with circulating LDL, they are in strict opposilion to the data obtained in most cell culture studies.

Mutant YAC coding for truncated human apolipoprotein B were used in this study to express these forms in transgenic mice. The results revealed that sequences C-terminal to apo B95 are necessary for efficient Lp(a) assembly, most likely via lysine binding sites on kringle 4 types 5 to 8 suggesting an opposite (C-terminal to N-terminal) orientation of the two involved proteins.
