Ophthalmology 1997, 105:1165-1169. Using the Medicare database of a total of 27,886 cases of partial-and full-thickness filtration surgeries performed between 1984 and 1990, the authors identified full-thickness procedures, concomitant vitrectomy, and inpatient hospitalization as increasing the risk of complications after surgery.

Adjunctive MMC significantly improved the filtration outcome of the primary glaucoma triple procedure in patients with one of three prognostic factors for filtration failure: African-American race, a preoperative IOP of 20 mm Hg or more on maximum tolerated medications, and being on two or more medications preoperatively.

    • 14 The AGIS Investigators: The Advanced Glaucoma Intervention Study (AGIS): 4: comparison of treatment outcomes within race: seven-year results. Ophthalmology 1998, 105:1146-1164. This National Eye Institute-sponsored, multicenter, randomized, controlled trial, initiated in 1988, compared the visual outcomes for two sequences of surgical glaucoma therapy: ATT versus TAT. Through 7 years of follow-up, the results suggest that, in order to minimize vision loss in patients with advanced glaucoma, the first surgical intervention should be ALT in blacks and trabeculectomy in whites.
