Use of the acute cardiac ischemia time-insensitive predictive instrument (ACI-TIPI) to assist with triage of patients with chest pain or other symptoms suggestive of acute cardiac ischemia: A multicenter, controlled clinical trial
-
12 Selker HP, Beshansky JR, Griffith JL, Aufderheide TP, Ballin DS, Bernard SA, et al.: Use of the acute cardiac ischemia time-insensitive predictive instrument (ACI-TIPI) to assist with triage of patients with chest pain or other symptoms suggestive of acute cardiac ischemia: a multicenter, controlled clinical trial. Ann Intern Med 1998, 129:845-855. In this study, 10,689 total patients in 10 hospitals were managed with and without use of the acute cardiac ischemia time insensitive predictive instrument printed onto the ECG. Use of the instrument resulted in a 6% reduction in the number of patients admitted without acute cardiac ischemia, a significant shift from the CCU to telemetry beds in patients with stable angina, and no reduction in CCU admissions for patients with acute MI or unstable angina.

Early echocardiography can predict cardiac events in emergency department patients with chest pain
-
22 Kontos MC, Arrowood JA, Paulsen WH, Nixon JV: Early echocardiography can predict cardiac events in emergency department patients with chest pain. Ann Emerg Med 1998, 31:550-557. Echocardiography performed within 4 hours of presentation in 260 patients with symptoms suggestive of myocardial ischemia was more sensitive (but less specific) than the electrocardiogram in predicting cardiac events and added significant incremental value to clinical variables and the electrocardiogram.

Chest Pain Evaluation in the Emergency Room (CHEER) Investigators. N Engl J Med 1998, 339:1882-1888. This randomized trial used a 6-hour monitoring period followed by cardiac stress testing and revealed significant cost-savings and non-compromise of patient safety with the abbreviated versus conventional hospital care.
