The pain threshold to repeated intramuscular stimulation was significantly lower for the patients with fibromyalgia compared to control group, and muscle pain evoked by hypertonic saline was of longer duration in fibromyalgia patients than in controls. This study supports the hypothesis that hyperalgesia in patients with fibromyalgia is due to an upregulation in the central nociceptive system.

Measurement of 24-hour growth hormone secretion in patients with fibromyalgia revealed significantly lower secretion of this hormone in patients in comparison to healthy controls. This finding was interpreted as indicating disturbed neuroendocrine function as a possible cause of fibromyalgia.

This important study has demonstrated that women with fibromyalgia and low IGF-1 levels experienced an improvement in their overall symptoms and number of tender points after 9 months of daily growth hormone therapy. These results suggest that a secondary growth hormone deficiency may be responsible for some of the symptoms of fibromyalgia.

Centers for Disease Control criteria for chronic fatigue syndrome were fulfilled by 42.3% of 71 individuals infected with Q fever 5 years earlier in the United Kingdom and in only 26% of controls. This study was interpreted as evidence that chronic fatigue syndrome may exist after an initial infection (Q fever), in a group of patients exposed only once to the organism, and in circumstances free of such confounding factors as lawsuits over compensation.

J Neurol Neurosurg Psychiatry 1998, 64:362-367. Muscle histometry in 105 patients with chronic fatigue syndrome generally did not show the changes expected as a result of inactivity. Patients with abnormal lactate responses to exercise, however, had a significantly lower proportion of mitochondria-rich type 1 muscle fibers.

Nearly two thirds of 137 patients with chronic fatigue syndrome reported an improvement on direct ratings of change after 1 year of follow-up. Poorer outcomes (fatigue and functional impairment) were predicted by illness duration, subjective cognitive difficulty, and somatic symptoms.
