Endoscopic sphincterotomy was performed in 1921 cases with an overall complication rate of 5.8%. The majority of complications were mild, requiring less than 3 days of hospitalization. Complication rates were lower for patients with normal bile ducts (< 9 mm) and age less than 60 years.

Gastrointest Endosc 1998, 48:457-464, Two hundred three patients who underwent a sphincterotomy for choledocholithiasis were followed for a mean of 6.2 years. Fifteen percent had recurrent biliary symptoms. The gallbladder left in situ and choledochal diameter were risk factors for recurrent symptoms, while duct size and presence of a periampullary diverticulum were risk factors for recurrent stones.

Late complications including stone recurrence, cholangitis, and cholecystitis occurred in 9.7% of patients.

Gastrointest Endosc 1998, 48:465-469. Four hundred ten patients who underwent an ES for choledocholithiasis were followed for a mean of 122 months. Stone recurrence-invariably of bilirubinate type-occurred in 12.3% of patients, suggesting that ablation of the sphincter might have a causative role in recurrent stones and symptoms.

Two hundred seventeen patients with symptomatic choledocholithiasis were evaluated for successful clearance of stones and complication rate. Successful clearance was achieved in 98% of cases with no mortality and a 5% complication rate. This compares favorably to laparoscopic surgery, which has an 86.4% clearance and a 10.1% complication rate.

Of the remaining patients 21% were found to have persistent CBD stones at a mean of 6.8 days after presentation with total bilirubin, ALT, and alkaline phosphatase levels being the best clinical predictors of retained stones.

Pancreatic sphincterotomies performed on 160 patients were found to be safe with a 1.8% complication rate but a significantly higher complication rate if concomitant pancreatic drainage was not performed with a stent or nasopancreatic drain. Sphincterotomy was determined to be efficacious in 64% of patients when used as primary therapy and in 70% when used as an adjunct to other therapeutic interventions.

Sixty-four patients with suspected choledocholithiasis underwent evaluation with both ERCP and EUS in a blinded fashion. EUS and ERCP success rates (98% vs 94%) and accuracy (94% vs 97%) were equivalent. The complication rate was significantly lower with EUS, and EUS was a more cost-effective strategy in patients with a low to moderate risk of having CBD stones.

Thirty-five patients with suspected acute biliary pancreatitis underwent transabdominal ultrasound, abdominal CT, ERCP, and EUS. EUS and ERCP both had a sensitivity of 100% for identifying CBD stones. EUS was able to identify further pancreatic pathology and had a 0% complication rate. The authors suggested EUS be used for diagnosis in acute biliary pancreatitis with ERCP limited to therapeutic cases where stones are identified.

Gut 1998, 42:703-707. Endoscopic management of occluded metal self-expanding biliary stents is compared in 44 occluded stents. Initial resolution of obstruction was found in 98% of cases but no significant difference in patency rates was seen when comparing placement of a second Wallstent (75 days), insertion of a plastic stent (90 days) or mechanical clearing of the original stent (34 days). The most cost-effective strategy was placement of a plastic stent.
