J Clin Invest 1998, 101:2029-2035. An early feature of apoptosis is the redistribution of phosphatidyl serine to the outer face of the plasma membrane. In this study, SSc sera positive for anti-EC antibodies but negative for anti-phospholipid antibodies were added to normal EC cultures. Phosphatidyl serine exposure, measured by annexin V binding, was detected following application of 6/8 SSc sera. This effect did not occur by means of Fas receptor binding, It was concluded that anti-EC antibodies in SSc might be pathogenic by inducing apoptosis.

This study followed earlier findings by this group that the three main SSc-associated autoantibodies (anti-RNAP III, anti-topo I and ACAs) were mutually exclusive. Using in-depth analysis of HLA-autoantibody associations, the following two possibilities were explored: first, the possibility that these groups represent three distinct disease entities, and, second, the possibility that these groups have a common origin which is modified by HLA specificity into the serologically defined disease subgroups. It was demonstrated that autoantibody associations with HLA class II alleles were not close enough to be wholly responsible for the observed mutual exclusivity. Therefore, the separate disease hypothesis was supported.

Chest 1998, 114:801-807. This paper compared the incidence of anti-topo I and anti-RNAP II antibodies in black and white patients with SSc. Anti-topo I, but not anti-RNAP II, antibodies were found to be associated with pulmonary involvement in both groups. Black race was also associated with decreased lung function. Although anti-topo I antibodies occurred more frequently in black patients, anti-topo I antibodies and black race appeared be independently associated with lung involvement in SSc.

Arthritis Rheum 1999, 42:465-474. Anti-topoisomerase antibody-positive SSc patients from four ethnic races (white, black, Japanese, and Native American Choctaw) were compared. Lung involvement was less common in white than in black or Japanese patients, Different epitopes of the topo I molecule were recognized by antibodies from the different groups. Furthermore, the immunogenetic associations of anti-topo I antibodies were different in the different races. It was concluded that ethnic background is an independent determinant of pulmonary involvement and survival in SSc.

Arthritis Rheum 1998, 41:1729-1737. This study sought to localize disease susceptibility genes for SSc in a Native American Choctaw population. This group has a very high prevalence of SSc, and there is evidence of a founder effect. Candidate genes were considered using microsatellite techniques. Alleles homologous to the murine tight skin-1 and -2 loci were analyzed for disease associations. A 2-cM haplotype on chromosome 15q that contains fibrillin I was found to be associated with SSc in the Choctaw population.

Arthritis Rheum 1999, 42:275-284. The purpose of this study was to determine the subunit specificities of anti-RNAP antibodies in SSc. The three multi-subunit RNAPs (I, II and III) share a number of sequence homologies, and some of the smaller subunits are common to two or all three enzymes. Fifteen different RNAP subunits, in different combinations, were found to be recognized by various serum samples. All anti-RNAP II-positive sera were found to recognize RNAP II-specific large subunits. Meanwhile, all sera precipitating both RNAPs I and III recognized the largest subunit of RNAP III. Some anti-RNAP I/III sera, and many anti-RNAP I/II/III sera, were also found to recognize some of the smaller shared subunits. The authors concluded that anti-RNAP antibodies in SSc sera recognize multiple subunits of RNAPs I, II and III, and that there is evidence of shared subunits and antigenic cross-reactivity.

The distribution of HLA class II and tumour necrosis factor alleles in the two groups was analyzed. A neutral or protective haplotype in idiopathic patients with anti-tope I antibodies was shown to be a susceptibility haplotype in silica-associated disease. It was concluded that different molecular mechanisms are involved in the development of idiopathic and silica-associated SSc.
