This is a prospective single-center study of the use of stem cell transplantation in 18 patients with CLL, including early stage disease. Stem cell harvesting was successful in 14 patients, transplantation was performed in 13 patients. Of note, engraftment was delayed in patients receiving bone marrow in contrast to peripheral blood stem cells. Clonal PCR was detected in three patients post-transplant. However, polyclonal PCR resulted in all other transplanted patients with a median follow-up of 19 months and 12 patients are in complete clinical remission.

This is a retrospective study of 50 heavily pretreated patients with Hodgkin's disease and non-Hodgkin's lymphoma, 23 of them with low-grade non-Hodgkin's lymphoma, who received autologous peripheral blood SCT. Factors influencing progenitor cell yield and platelet recovery were investigated, demonstrating regimens containing cytarabine and cisplatin to show significantly less progenitor yield and slower platelet recovery after transplantation.

These authors report retrospectively on the results of autologous peripheral blood stem cell transplantation in 120 patients with non-Hodgkin's lymphoma (aggressive and low-grade) treated at several disease stages. Patients who were transplanted in first remission had a significantly higher event-free survival than patients in first partial remission or second complete remission. Additional radiotherapy and graft purging with mafosfamide were associated with a higher event-free survival.

The authors suggest that CD6+ T cell depletion is associated with a lower risk of transplant-related complications.

Concludes that a significant proportion of patients have clonal hematopoiesis at the time of autologous SCT, which is predictitive for the development of t-myelodysplastic syndrome or acute myeloid leukemia.

The authors suggest advantage of autologous transplantation because of the high mortality rate after allogeneic transplantation.

Bone Marrow Transplant 1999, 23:329-333. This is a small study of 13 patients with mainly advanced low-grade, intermediate-grade and aggressive lymphoma undergoing allogeneic SCT. Complete remission was achieved in 8 patients. The overall survival rate in this study was 67% and the median time of event-free survival was 102 months. Of note, donor lymphocyte infusion after relapse of two patients with low-grade lymphoma resulted in one partial and one complete remission which, however, was associated with grade IV acute graft-versus-host disease.

J Clin Oncol 1998, 16:2817-2824. This single-center study demonstrates a direct graft-versus-lymphoma effect in 15 patients with CLL and low-grade or transformed low-grade non-Hodgkin's lymphoma. Patients received primarily a non-ablative immunosuppressive chemotherapy protocol followed by allogeneic stem cell infusion from HLA-identical siblings and in some cases donor lymphocyte infusion 2 to 3 months later. Eleven patients were engrafted successfully and 8 of 11 patients achieved a complete remission. Remission rate was significantly higher in patients with chemosensitive disease than in patients with refractory disease. However, acute or chronic graft-versus-host disease occurred in over 50% and was lethal in two patients. Whether there might be an advantage in event-free survival in contrast to non-transplanted patients remains to be shown.
