Gastroenterology 1997, 113:1560-1569. This study provides a mechanism for the IgE-, mast cell-dependent neutrophil infiltration of the stomach of mice. During this late-phase allergic response, mast cells are shown to be at least one source of TNF-α mRNA.

The authors now show that increased concentrations of IgE also upregulates FcεRI expression and increases inflammatory responses.

Although IgE immunoglobulins are known as the traditional activators of mast cells, they are not the only such molecules capable of stimulating mast cell responses. This study shows that complement is critical to the mast cell-dependent clearance of bacterial peritonitis and, importantly, demonstrate a protective effect of mast cells.

Because this phenotype is related to the overproduction of IgE, therapeutic options may be expanded to affect the production or attachment of IgE molecules. Although these specific types of treatments have not yet been attempted in gastrointestinal allergic disease, their use may arrive as more is understood about the pathophysiology and genetics of gastrointestinal atopic diseases.

Their findings suggest that allergic conditions may arise from the over expression of the effects of IL-4. Whether this finding, as well as the previous mutations identified, are related to gastrointestinal allergic disorders remains to be proven.

Gut 1997, 40:745-753. Seventy adult patients with suspected food allergy had intracecal challenge with food antigens. A wheal and flare reaction was seen in 77% of index patients and none of the control subjects. There was no significant relationship between skin test and colonoscopic allergen provocation test results to the same antigen. Histologically there was mast cell and eosinophil degranulation. Although skin testing and serum immune testing prove to be nonspecific in the screening of patients for food allergy, colonoscopic allergen provocation testing provides clinicians with a means of eliciting antigen-specific gastrointestinal reactions to various food proteins.
