This retrospective study suggests that HPN improves nutritional status and performance in patients surviving at least 3 months. Karnofsky score at the time of starting HPN appears to be a reliable marker of life expectancy. This performance score is not, however, a specific tool for the assessment of QOL.

    • Howard L, Malone M. Clinical outcome of geriatric patients in the United States receiving home parenteral and enteral nutrition. Am J Clin Nutr 1997; 66:1364-1370. This study shows, as expected, that geriatric patients on HPN have a poorer short-term life expectancy than younger patients, but an acceptable overall outcome. On the contrary, the study underlines ethical issues raised by the decision to use HEN, because this technique is associated with a poor survival rate in geriatric patients.

    • Richards DM, Irving MH. Assessing the quality of life of patients with intestinal failure on home parenteral nutrition. Gut 1997; 40:218-222. This study is one of the few attempts to assess the results of home parenteral nutrition in terms of QOL using formal validated scores. It shows that QOL is good in cases of non-malignant underlying disease, whereas lower scores are observed in opiate analgesic-addicted patients and an ageing population.
