The authors describe 15 patients with systemic lupus erythematosus who had suffered 19 episodes of DAH and compared them with 57 supported cases from the literature. In eight of 10 patients in whom lung tissue was available, pulmonary capillaritis accompanied the DAH. The overall mortality was 53%. Corticosteroids were the mainstay of therapy.

In a succinct editorial the author draws our attention to the pulmonary findings in antiphospholipid syndrome. In a patient with known antiphospholipid syndrome, the presence of diffuse lung infiltrate or an acute respiratory distress syndrome-like picture should lead one to think about DAH. Lung biopsy is not needed in cases with typical clinical features. Anticoagulants are the mainstay of treatment.
