Results may herald an eventual change in routine radiologic practice.

Another important study of CT image manipulation, adding to our experience with maximum intensity projection and pioneering the application of minimum intensity projection. Taken together, the results suggest that the two techniques may eventually play a major diagnostic role in small airways disease by enhancing the sometimes subtle CT signs of micronodularity (using maximum intensity projection) and mosaic perfusion (using minimum intensity projection).

The variable spectrum of CT abnormalities in constrictive bronchiolitis included micronodules and branching structures, as well as typical mosaic perfusion.

Mosaic perfusion on expiratory CT scanning was the major determinant of airflow obstruction, but bronchial wall thickening and lack of global change in cross-sectional area between inspiratory and expiratory CT scanning were also functionally significant and can be regarded as ancillary CT signs.

The authors' conclusion that mosaic perfusion on CT scan discriminates between pulmonary thromboembolism and other pulmonary disorders is an overstatement; patients with small airways disease were not included in the study.

A study addressing the problem of discriminating between causes of mosaic perfusion on CT scan in 70 patients with diffuse infiltrative lung disease (with patchy ground-glass attenuation), airways disease (with patchy decreased attenuation), or pulmonary vascular disease. The radiologic observers identified the first two categories accurately but the mosaic perfusion of pulmonary vascular disease was frequently misinterpreted as indicative of primary lung disease. These results highlight the difficulties of interpreting a CT pattern of mosaic perfusion in isolation, without taking the clinical presentation into account.

This study documents CT signs typical of constrictive bronchiolitis, including marked enhancement of mosaic perfusion on expiratory CT scan. Mosaic perfusion correlated more strongly than large airway abnormalities with airflow obstruction on lung function evaluation.

A description of serial change on inspiratory CT scanning after lung transplantation in 13 patients, including eight who developed obliterative bronchiolitis. Volume loss and decreases in bronchial and vascular markings were the earliest CT abnormalities; the more striking CT manifestations of bronchiolitis (mosaic perfusion and bronchiectasis) occurred only when disease was well established. The study suggests that inspiratory CT scanning does not provide sufficient signal to identify early obliterative bronchiolitis consistently.

A description of expiratory CT abnormalities in established obliterative bronchiolitis following lung transplantation. Mosaic perfusion was present in six of 15 patients with obliterative bronchiolitis but also in four of 18 control subjects; however, the combination of mosaic perfusion and bronchiectasis was seen exclusively in obliterative bronchiolitis. The lack of mosaic perfusion in the majority of patients suggests that expiratory CT scanning should be combined with other diagnostic modalities but should not be relied on in isolation in the evaluation of transplant patients.

A description of striking regional decreased attenuation (mosaic perfusion) on expiratory CT scan in 22 patients with subacute hypersensitivity pneumonitis. This CT sign was obvious in the great majority of patients and, although less extensive than ground-glass attenuation, was the major CT determinant of functional impairment on multivariate analysis. The authors argue that decreased attenuation on CT scan is indicative of the bronchiolitic component of hypersensitivity pneumonitis. The study suggests that when associated with widespread ground-glass attenuation, decreased attenuation can be regarded as a cardinal CT sign of hypersensitivity pneumonitis.
