    • Gilbert MS, Aledort LM, Seremetis S, Needleman B, Oloumi G, Foster A: • Long term evaluation of septic arthritis in hemophilic patients. Clin Orthop 1996, 328:54-59. Ten of 539 patients developed 15 episodes of pyoarthritis in an 8-year period. Three had HIV infection. No classic symptoms of septic arthritis were seen, and diagnosis was delayed in five patients. All but one responded to antibiotics. Six patients died of AIDS from 2 to 109 months. Three patients were alive at 29 months, 86 month, and 96 months, respectively. Early diagnosis with a high suspicion is the best predictor with respect to joint function.

The authors document the rheumatologic manifestations in patients with MDS and compared them with patients with myeloproliferative disorders. A prevalence of 7% of arthritis and vasculitis in patients with MDS was found. A seronegative polyarthritis (rheumatoid-like) was associated with MDS. It responded to low doses of prednisone.

They emphasize the notion that rheumatic manifestations may be the presenting feature of lymphoproliferative disorders. In addition, they discuss that therapy or disease complications may mimic rheumatic syndromes.

Authors reported a patient with POEMS syndrome with biclonal gammopathy who was treated with tretinoin, 90 mg/d for 50 days, followed by no treatment for 70 days; a second course of tretinoin was administered for 180 days. Serum levels of interleukins-6 and -1β and TNF-α normalized within 7 days after the first administration of tretinoin. Platelet and gammopathy paralleled the changes in the cytokine levels.

The case of a patient with rheumatoid arthritis who received an allogenic BMT for gold induced aplastic anemia with a follow-up of 13 years is reported. Although the rheumatoid arthritis was in remission for 2 years after the transplant they present clinical, radiologic, and laboratory evidence of progression from 3 to 13 years after the BMT. Implications with respect to pathogenesis and the future of immunotherapies are discussed.
