Ten of 539 patients developed 15 episodes of pyoarthritis in an 8-year period. Three had HIV infection. No classic symptoms of septic arthritis were seen, and diagnosis was delayed in five patients. All but one responded to antibiotics. Six patients died of AIDS from 2 to 109 months. Three patients were alive at 29 months, 86 month, and 96 months, respectively. Early diagnosis with a high suspicion is the best predictor with respect to joint function.

5. Chandran G, Ahern MJ: Rheumatic manifestations of the myelodysplastic syndromes: a comparative study. Aust N Z J Med 1996, 26:683-688. The authors document the rheumatologic manifestations in patients with MDS and compared them with patients with myeloproliferative disorders. A prevalence of 7% of arthritis and vasculitis in patients with MDS was found. A seronegative polyarthritis (rheumatoid-like) was associated with MDS. It responded to low doses of prednisone.

A good review of rheumatic manifestations of dysproteinemia and lymphoproliferative disorders including POEMS syndrome, multiple myeloma, and lymphoma among others. They emphasize the notion that rheumatic manifestations may be the presenting feature of lymphoproliferative disorders. In addition, they discuss that therapy or disease complications may mimic rheumatic syndromes.

Authors reported a patient with POEMS syndrome with biclonal gammopathy who was treated with tretinoin, 90 mg/d for 50 days, followed by no treatment for 70 days; a second course of tretinoin was administered for 180 days. Serum levels of interleukins-6 and -1β and TNF-α normalized within 7 days after the first administration of tretinoin. Platelet and gammopathy paralleled the changes in the cytokine levels.

A good review showing the available evidence that links rheumatoid arthritis as a bone marrow disorder. Authors emphasize the role of synoviocytes as the most important cells in rheumatoid arthritis panus. Authors also reviewed the impressive evidence relating the effects of BMT on arthritis in humans and in experimental models.

25. McKendry RJR, Huebsch L, Leclair B: Progression of rheumatoid arthritis following bone marrow transplantation: a case report with 13-year follow-up. Arthritis Rheum 1996, 39:1246-1253. The case of a patient with rheumatoid arthritis who received an allogenic BMT for gold induced aplastic anemia with a follow-up of 13 years is reported. Although the rheumatoid arthritis was in remission for 2 years after the transplant they present clinical, radiologic, and laboratory evidence of progression from 3 to 13 years after the BMT. Implications with respect to pathogenesis and the future of immunotherapies are discussed.
