A prospective study in which 69 patients with stroke or transient ischemic attack were evaluated with cardiac stress test to assess for CAD. Twenty-four out of 69 (35%) patients were noted to have asymptomatic CAD. Large artery cerebrovascular disease, smoking, veteran status, and possibly left ventricular hypertrophy were predictive of asymptomatic CAD on the multivariate analysis.

A prospective clinical study in which seven patients with confirmed infarct (on magnetic resonance imaging) in the left insular cortex were investigated with computerized autonomie platform monitoring, followed by power spectral analysis to assess the cardiac autonomie tone. The results suggest that the left insular lesion may shift the autonomic balance towards increased sympathetic tone and decreased parasympathetic tone. Such a change has been shown to predispose to cardiac dysrrhythmia.

A prospective study of 140 patients with stroke and PFO assessed by transesophageal echocardiogram followed for a mean period of 36 months. The rate of stroke or death was 2.4% whereas that for neurologic event (stroke or transient ischemic attack) was 3.8% per year. Using multivariate analysis the following factors were found to be predictive of stroke recurrence in PFO patients: interatrial communication, posterior cerebral artery stroke, coexisting cause of stroke and history of migraine.

On the basis of the findings, appropriate modifications of surgical techniques were made. An overall perioperative stroke rate of 0.76% was reported. Age, diabetes, increased serum triglycerides and very low density lipoprotein, and aortic calcification were identified to be the risk factors for perioperative stroke on multivariate analysis.

The results showed that the simultaneous procedure was associated with increased neurologic morbidity.
