J Clin Pathol Mol Pathol 1997; 50:45-50. The authors used a combination of PCR reactions directed against sensitive and resistant penicillin-binding protein genes. They were able to detect susceptible strains easily, but many isolates that are resistant were not correctly identified as resistant. This is because the resistant PCR reaction can is only positive if the unknown isolates share seguence homology with these primers. Resistant isolates are highly heterogeneous and it is inevitable that many resistant strains will not be detected by this approach.

Sensitive patterns could be distinguished from resistant patterns and the RFLPs were used to follow the spread of a serotype 9 resistant strain that was causing a hospital outbreak.

This is an extension of a previous observation that a portion of the IS611O insertion sequence shares homology with DNA from non-tuberculosis species. As this insertion sequence is commonly used as a target for diagnostic PCR reactions such homology may result in false positive results. In this study, the authors identified that the homologous DNA was present in multiple copies in the genomic DNA of non-tuberculosis mycobacteria. This indicated that mycobactena may possess a family of IS3 insertion sequences.

Known resistance mutations could be detected rapidly for streptomycin, rifampicin and isolniazid. A proportion of resistant strains were not identified by molecular techniques.

An IS6110 PCR was used on sequential specimens from patients with pulmonary tuberculosis. Patients who subsequently relapsed were more likely to remain PCR-positive after 6 months of treatment. This was a useful study, but care must be taken that the results are not generalized inappropriately. Only sequential PCR assays can give an indication of the response to treatment, the significance of a single positive or negative sample cannot be interpreted during the course of a patient's therapy.

A nested PCR protocol was used to detect C. pneumoniae in sputum from patients with community-acquired pneumonia in Sweden. The PCR proved almost as sensitive as serological methods in making a diagnosis. This represents an improved diagnostic yield, and may allow further epidemiological investigations to establish the role of this pathogen in community-acquired pneumonia.

PCR targeted on the HL1/HLR primer pair was used to investigate lavage fluid prospectively. The population included a number of HIV-seropositive individuals. C. pneumoniae was found as a single pathogen in many hospital patients. This included the HIV-infected individuals who would otherwise have been difficult to diagnose by conventional serological methods.
