Kershner RM: Clear corneal cataract surgery and the correction of myopia, hyperopia, and astigmatism. Ophthalmology 1997. 104:381-389. A survey of 690 consecutive cataract procedures with keratolenticuloplasty revealed that clear corneal incision (CCI) procedures combined in some cases with arcurate keratotomies effectively corrected preexisting myopia, hyperopia, and astigmatism.

Six pairs of piggyback lenses were implanted in extremely short eyes. Calculations were performed with three current (Holladay, Hoffer Q, SRK/T) and two older (SRK1, SRK2) formulas. Even the newer formulas were found to be insufficient for IOL calculation at extremely short axial length.

A retrospective study of 32 highly myopic eyes (27 patients)-axial lengths from 31 to 35 mm-with cataract surgery and implantation of posterior chamber lenses (PCL) from -1 to -8 D was reported. The results showed that cataract surgery can be performed in highly myopic eyes without intraoperative complications, A PCL IOL should be implanted for postoperative refraction and intraocular stability, even if negative lens power is required.
