A survey of 690 consecutive cataract procedures with keratolenticuloplasty revealed that clear corneal incision (CCI) procedures combined in some cases with arcurate keratotomies effectively corrected preexisting myopia, hyperopia, and astigmatism.

Six pairs of piggyback lenses were implanted in extremely short eyes. Calculations were performed with three current (Holladay, Hoffer Q, SRK/T) and two older (SRK1, SRK2) formulas. Even the newer formulas were found to be insufficient for IOL calculation at extremely short axial length.

The author evaluated the lens haptic plane (LHP) concept for IOL calculation. The predictability in postoperative refraction found by applying the LHP concept and paraxial ray tracing was excellent.

14. Kohnen S, Brauweiler P: First results of cataract surgery and implantation of negative power intraocular lenses in highly myopic eyes. J Cataract Refract Surg 1996, 22:416-420. A retrospective study of 32 highly myopic eyes (27 patients)-axial lengths from 31 to 35 mm-with cataract surgery and implantation of posterior chamber lenses (PCL) from -1 to -8 D was reported. The results showed that cataract surgery can be performed in highly myopic eyes without intraoperative complications, A PCL IOL should be implanted for postoperative refraction and intraocular stability, even if negative lens power is required.

A retrospective study of 165 clear lensectomies in patients with myopia of -12 D or greater and different surgical techniques showed a high rate of postoperative retinal detachment.
18. Colin J, Robinet A: Clear lensectomy and implantation of low-power posterior chamber intraocular lens for correction of high myopia. Ophthalmology 1997, 104:73-77. The authors found less detachment in their 4-year follow-up prospective study. All the patients had preoperative treatment of retinal degeneration and were operated with phacoemulsification.
