This study suggests that patients who meet the low-risk criteria (no microbiologic diagnosis and signs of bone marrow recovery) can be safely discharged from the hospital irrespective of their acute lymphoblastic leukemia or the duration of neutropenia. This small study must be validated in larger controlled studies before this practice should be widely embraced.

The authors reported that in pediatric patients with established febrile neutropenic episodes the use of G-CSF produced a small but significant reduction in duration of antibiotic therapy and hospital stay (median, 5 vs 6 d; P = .02, and median, 5 vs 7 d; P = 0.2, respectively). The difference in cost was negligible. The magnitude of the observed differences hardly justifies widespread use of G-CSF for adjunct therapy for fever and neutropenia.

In this study of 164 patients with acute lymphoblastic leukemia, G-CSF treatment initiated 1 day after completion of remission-induction therapy did not significantly influence outcome. The authors did not report a significant reduction in the rate of hospitalization for febrile neutropenic events, prolonged survival, or reduction in the cost of supportive care.

Granulocyte colony-stimulating factor markedly shortened the time of recovery of granulocytes in patients with acute myeloid leukemia after intensive consolidation therapy (20.5 days for patients receiving G-CSF compared to 31 days for patients not treated with G-CSF, P < 0.001). The reduction in hospitalization was reduced from 97% to 85% (P = 0.05), but the duration of complete remission and the overall survival were similar in both groups.

Although neutropenia was shortened, the study did not demonstrate practical clinical benefit of this strategy.

The use of G-CSF reduced the incidence of febrile neutropenic episodes and documented infections (12% vs 42% in control subjects, P < 0.05 and 40% vs 77%, P < 0.05, respectively).
